All such detailed medical infomation should really go to medically qualified personel who will hold such data under strict medical in confidence rules. (MSA have their own medical section) You could request a medical report, which again can be vetted by driver before release to you.(Data Protection does apply but in addition there are Medical Reports and Records legislation to also take into account).
