I have no idea how often these do develop faults. The only problem I have had is not setting it properly. When you set it to record the light flashes so it is possible to check that you have set it OK but when it is tipping down with rain then maybe you get it wrong.
Fairly easy though for CoC to check - just sets it to record in the office and then puts the card in the computer - if it recorded then operator error and he could exclude. Maybe the same if you have not charged the battery.
