Does that mean that the problem has only occurred on ONE occasion....? I.e., have they just 'come undone' once....?

If they've done it repeatedly..... it's the elongated holes.

If they've done it once..... you forgot to tighten them... no matter what you think.... it's as simple as that!

I thought you'd had a repeated problem with the same wheel/hub.
