As Simon states, failure can originate from a very small imperfection, hence my doubts on you drilling and machining the new position with a sufficient degree of perfection.

You should regularly check the unused key way if there is one on the axle as it is a good place for a crack to propagate from as there is no support over the key way in the form of a hub or carrier.

Use a powerful magnifier to carefully check for cracks starting from the holes.

If you remove an axle, try and get the grub screws back in the same marks when you reassemble things and don't over tighten the grub screws.
