YES!!!
The licence application form is critical and downloading from the MSA website is not a good idea as each form has a unique reference number, which is required to complete the test documentation. You could always talk to the MSA to see if they could just send you the form and not the other gubbins. No doubt at some cost, but probably less than the Ł42.
