Could be a cracked assembly backplate.

If you mis-aligned the assembly on the shaft and the key was not properly aligned you put the whole assembly under huge stress. When you tighten the clutch onto the shaft all is well in that the clutch works but then eventually the backplate simply gives up the ghost and cracks.

I've seen this twice.

If it is this you should be able to see the cracks around the central "hole" in the clutch when you remove it from the shaft.

Repair is simply to replace the plate.
