My reading of the situation is that any organisation would see this "issue" as a risk needing urgent attention because :

Risk ( size of) = Number of occurrences X Impact

The number of occurences has been highlighted by petition ( this is flipping scooping and serious injury) the impact is potential disaster and already existing injuries.

WE DONT HAVE TO MENTION ZIP NOSE CONE This may or may not be the cause.(However a blind man with a stick in his eye would see that it was).

The MSA stating "There is no clear evidence to hand" does NOT remove any obligation to Risk assess the reported issue as a VERY high priority.

I am sure their insurance and risk management partner would also see it this way as the potential cost to them is enormous !!! If there was obvious action that could be taken swiftly to reduce this risk i am sure they would support this without concern to commercial interests.
