I have had this before and this might help.
Take the chain off and spin the axle with the sprocket attached on the carrier. You might notice the sprocket moves up and down very slightly which then means the chain will go slack/tight. What i have found is the sprocket ( the cheap gold ones seem to be the worst) doesn't spigot onto the carrier accurately so its possible to bolt the sprocket up with all the play in one direction hence the run out.
Also i had one carrier where if you tightened one of the clamp bolts to the axle first before the other it actually pulled it slightly of line..
worth a look..
