Here's an interesting one. You know some traffic lights remain on red until triggered by a motion sensor which is activated when a car arrives along. What if that motion sensor fails to trigger the light ? How long are you supposed to remain on red ? In other words, are you allowed to traverse a red light if the traffic lights themselves are malfunctioning ?
