Theres also some good debates in the house of commons over this. One is four hours long and all are informative.

There where massive mistakes made in effectively organising entry and crowd direction. I think the biggest issue was the initial reaction by the authorities to suppose a cause without an investigation.

I would imagine there was some drunkenness, rowdy behaviour and clearly an issue outside the ground by misinforming people about the time to get there and the entry arrangements but blaming a tanked up crowd is an easy way out to cover up a fair few failings.

If you put on big events like that you have to take steps to reduce the risk of injury from overcrowding as far as is reasonably practicable. That clearly wasn't done.
