We had a similar problem recently with brakes that didn't work at the track; though they weren't OTK.

We traced it to the pin that transfers the load from the brake lever to the master push rod.

The pin had lost its circlip and slipped half out of the hole. Under load it slipped further out, but when the pressure was released the rubber boot surrounding if pushed it back into the hole.

Having found it once, we then found several brakes that had lost the circlip. Standard maintenance was to service the master cylinder quite frequently, now I check the clevis pin as part of my pre race day schedule.
