Once you hit 18 you need a medical. then if memory is correct you self declare your fitness till you are 45, then you need a medical each year.

Medical consists of

Eye sight
Tap the knee
Pee in a pot
a few questions

relieve you of between Ł50 and Ł150 depending on your doctor.
