Could you not have the bolt and nut the other way round (i.e. the nut outboard) and use a bolt with a small volume head (such as a button caphead). We've had the same issue and this was the easiest solution we came up with.

I wouldn't take this as gospel though as we're very new to this and may well be doing things wrong!
