Get a thin flat file, approx 1mm, and increase the depth of the slot down the length of the rear hub. This will allow more surface area on the hub inner surface to clamp onto the axle. If you look at the slot machined down the hub you will see how it is deeper on the outer diameter of the hub compared to the inner diameter. This is due to the use of a circular cutter moving down the hub and of course the curve of the cutter will remain. File the inner curve down to a point level with the outer slot level. This will fix the problem as it did for us on 30mm hubs. Of course keep axle and inner hub surfaces degreased.
