Do you store the kart in an upright position?
If so, you must tie the brake pedal up with a cable tie to the front bar.
I've never found a problem with the OTK brake but the caliper must be clean on the pistons and bore.
Keep at least a 1/3 fluid level in the reservoirs while using the bleed tool.
Raising the rear of the kart and making sure there is no loop where air can collect can help.
Don't pump the brake, just let the fluid flow under gravity.
Make sure you can see the pads retracting slightly when you release the brake.
Does the pedal connecting rod allow the master cylinder pistons to return to full rest position?
